Summary of Complaint

I WAS DRIVING DOWNHILL, CAME AROUND A BLIND CURVE AND CARS WERE BACKED UP REALLY HIGH ON THE STREET. I REAR-ENDED THE CAR THAT WAS AT A DEAD STOP IN FRONT OF ME AS I TURNED THE CORNER. UPON STARTING THE CAR ONE MORNING I NOTICED THE SPEEDOMETER, FUEL AND GEAR INDICATOR DIGITAL READOUTS WERE NOT WORKING. I RESEARCHED THE PROBLEM ON THE INTERNET AND NARROWED THE PROBLEM TO A FAULTY COMBINATION METER (BAD SOLDER). THIS PROBLEM HAS BEEN OCCURRING WITH NUMEROUS OTHER PRIUS OWNERS ACCORDING TO FORUMS AND SUCH. I PURCHASED THE CAR AT A FORD DEALERSHIP AND ALSO PURCHASED THEIR FORD ESP BASE CARE PLAN. CLEARLY THE PLAN STATED THE SPEEDOMETER/ODOMETER, ELECTRICAL OR MECHANICAL PROBLEMS WERE COVERED. HOWEVER THE AGENT AT THE EXTENDED WARRANTY OFFICE DENIED THE CLAIM. SO MY CAR SITS AT A TOYOTA DEALERSHIP WITH THE DASH APART AND THE APPRAISED VALUE OF THE CAR LESS $2000.00 ACCORDING TO THE SALES DEPT. BECAUSE THE MILEAGE IS UNKNOWN. TOYOTA HAS YET TO DECLARE A RECALL FOR THE PROBLEM WHICH COULD RESULT IN DEATH IN SOME CASES IF NOT PROPERLY ADDRESSED. I HAVE TWO CHILDREN AND VALUE THEIR LIVES. Summary of Complaint

STARTING AROUND MAY,2005, WHEN VEHICLE WAS STARTED WITH THE BATTERY INDICATOR NOT QUITE AT FULL CAPACITY, LETS SAY SEVEN 'BARS', JUST PUTTING THE CAR IN REVERSE WOULD BEGIN THE GRADUAL DECLINE OF CHARGE. IF DRIVING UP A FRWY RAMP, THE INDICATOR WOULD SHOW THE CHARGE HAD DROPPED INTO THE 'RED' ZONE. AT THIS TIME THE ENGINE WOULD SOUND LIKE THE RPMS WERE ABOUT 4,000, A REAL REVVING SOUND. SLOWLY ,WITH A STEADY LEVEL DRIVE THE BATTERY WOULD RECHARGE AND LOOK AS IF IT WERE NORMAL. THERE WAS NO STALLING OF ENGINE BUT I RARELY DRIVE AS SLOW AS 65 MPH! THE WHOLE PROCESS, HAPPENING AT LEAST ONCE A DAY WAS VERY NERVE WRACKING...ESPECIALLY IN THE MOHAVE DESERT WITH MANY 18-WHEELERS SURROUNDING THE CAR. THE FIRST TIME IT HAPPENED I IMMEDIATELY CALLED TOYOTA OF TEMECULA VALLEY. I WAS TOLD THAT THIS WAS NOT GOOD AND TO BRING THE CAR IN FOR DIAGNOSIS. WHEN I BROUGHT IT IN, A 'TECHNICIAN' LOOKED AT THE INDICATOR AND SAID IT WAS OK AND NOT WORRY ABOUT IT. I HAD A DIFFICULT TIME BELIEVING THAT AND MADE AN APPT. WITH TOYOTA OF EL CAJON. WHEN THEIR TECHNICIAN USED A DIAGNOSTIC SCANTOOL HE ALSO SAID HE COULD NOT FIND A PROBLEM. I DID NOT BELIEVE HIM OR THE SERVICE $80/HR. GUY AND SAID THERE WAS EITHER SOMETHING WRONG WITH THEIR DIAGNOSTIC TOOLS AND/OR SOMETHING WRONG WITH THEM TO THINK I WOULD GO ALONG WITH THEIR STORY. I HAD NO IDEA HOW RIGHT I WAS UNTIL I READ ABOUT THE DEFECT INVESTIGATION ON YOUR WEBSITE. MY PRIUS HAS NOT STALLED (YET) BUT IT HAS ALL THE PROBLEMS LEADING UP TO IT. THE DEALERSHIP CLAIMS, ON THE WORK RECEIPT THEY GAVE ME, TO HAVE RECALCULATED THE HV ECU WITH THE 40D CODE AND THE PROBLEM SEEMS TO BE WORSE. 1. I WAS DRIVING AROUND 30 TO 40 MPH AND THE GAS ENGINE FAILED SUDDENLY DURING THE COURSE OF NORMAL DRIVING, LEAVING ONLY THE ELECTRICAL ENGINE RUNNING. AT THE SAME TIME THE ENGINE FAILED, MANY LIGHTS TURNED ON ON THE DASHBOARD. 2. I WAS ON A CANYON ROAD IN THE LOS ANGELES MOUNTAINS WHEN IT FAILED, WHICH WAS A SINGLE LANE ROAD EACH WAY WITH NOWHERE TO PULL OFF. LUCKILY, I HAD JUST REACHED ON OF THE CRESTS IN THE CANYON WHEN THE ENGINE FAILED, SO WITH THE ELECTRICAL ENGINE I WAS ABLE TO COAST TO A SIDE STREET. ONCE ON THE SIDE STREET, THE ELECTRICAL ENGINE DIED QUICKLY, PROBABLY BECAUSE OF THE STEEP SLOPE UPWARDS OF THE STREET. 3. I TRIED TO TURN OFF THE CAR AND IT WOULDN'T TURN OFF, SO I POPPED THE HOOD AND GOT OUT TO SEE IF THERE WAS AN OBVIOUS PROBLEM. FINDING NO PROBLEM I COULD SEE I WENT BACK INSIDE THE CAR AND HELD DOWN THE POWER BUTTON UNTIL THE VEHICLE TURNED OFF FINALLY. WHEN THE VEHICLE TURNED BACK ON, THE ENGINE SEMPT TO WORK NORMALLY, AND I IMMEDIATELY WENT TO THE NEAREST TOYOTA DEALER. THEY SAID THEY HAD HEARD OF THE PROBLEM ONCE OR TWICE BEFORE AND IT REQUIRES A SOFTWARE UPDATE TO THE CAR. I WAS WEARY OF THEIR PROPOSED SOLUTION SINCE IT IS SUCH A DANGEROUS PROBLEM, BUT THAT'S ALL THEY SAID THEY COULD DO, SO I TOLD THEM TO GO AHEAD.

UNFORTUNATELY MY CAR IS 2004 AND DOES NOT GET THE BENEFIT OF THE EXTENSION. THE COST TO REPAIR IS ALMOST $4,000. TOYOTA KNOWS OF THIS ISSUE AND SHOULD REPAIR MY CAR AS THEY ARE WITH THE OTHER YEARS 2010-2015. THIS IS A DRIVER SAFETY ISSUE.

ABOUT 1.5 YEARS AGO DURING A VACATION. MY CAR DECIDED TO NOT SHUT OFF. I DROVE TO THE NEAREST DEALERSHIP THAT WAS ABOUT THE CLOSE. THEY ASSISTED AND SAID TO HOLD THE POWER BUTTON DOWN UNTIL IT SHUTS OFF THEN TURN IT BACK ON. THIS WAS A DEALERSHIP IN GEORGIA. THE MECHANIC HAD HEARD OF THE PROBLEM BEFORE. I RECENTLY HAD THE FRONT DASH LIGHT GO COMPLETELY OUT. I DID NOT HAVE AN INSTRUMENT PANEL AT ALL. THE WAS NOT A WAY TO SEE MPH, GAS GAUGE, WARNING LIGHTS, NOTHING. THE CENTER MONITOR STAYED ON. IT TOOK 2 DAYS BUT IT CAME BACK ON. A WEEK LATER IT HAPPENED AGAIN AND AGAIN CAME BACK ON. WARNING LIGHTS CAME ON AND I DROVE TO DEALERSHIP. IT WAS THE 2ND WATER PUMP GOING OUT. AS I WAS LEAVING MY DASH LIGHTS WENT OUT AGAIN. I TOOK IT BACK INTO THE PRIUS DEALERSHIP. THEY HAD PAPERWORK STATING THERE HAD BEEN ISSUES WITH THE METERED DASH BEFORE. IS THERE A RECALL ON IT OR NOT? I HAD TO HAVE A CAR AND WAS ALLOWED TO LEAVE WITH THE DASH NOT WORKING. I DROVE TO A GAS STATION. AFTER FILLING WITH GAS THE DASH LIGHTS CAME ON AGAIN. I HAD TO PAY FOR THE METERED DASH PART TO BE ORDERED UP FRONT BECAUSE THE MECHANIC SAID IT MAY COME BACK ON. THERE SHOULD BE A RECALL ON THIS AS IT IS AN ONGOING ISSUE. THE MECHANIC CHECKED THE SHAFT AND SPLINES FOR DAMAGE AND FOUND NONE, THEN PROCEEDED TO REATTACH BOLT. I WAS ALSO INFORMED BY THIS MECHANIC THAT TOYOTA HAD TWO RELATED RECALLS: ONE FOR THE STEERING SHAFT, AND ONE FOR THE INTERMEDIATE STEERING SHAFT EXTENSION. BOTH OF THESE RECALLS HAD BEEN ADDRESSED AND REPLACEMENT PARTS INSTALLED FOR MY VEHICLE. IT DOES NOT HAPPEN OFTEN BUT IT IS REGULAR ENOUGH THAT THIS HAS ALWAYS CONCERNED ME VERY MUCH. TODAY I WAS FOLLOWING A CAR WHO SLAMMED ON THEIR BRAKES VERY SUDDENLY AND THEREFOR I ALSO HAD TO SLAM ON MY BRAKES. THIS SAME ISSUE OCCURRED AND I DID NOT HAVE TIME TO RELEASE THE BRAKES AND PRESS THEM AGAIN CAUSING ME TO HIT THE BACK OF THE CAR THAT I WAS FOLLOWING. I MAINTAIN MY CARS VERY WELL AND THIS CAR WAS IN TOP CONDITION WITH BRAND NEW TIRES. THE BRAKES DO NOT EVEN ACTIVATE ENOUGH TO SQUEAL OR LEAVE ANY RUBBER BEHIND. THERE IS NO WAY THAT I COULD POSSIBLY HAVE STOPPED IN TIME. THIS IS NOT SAFE AND NEEDS TO BE RESOLVED FOR OTHER PEOPLE WHO HAVE THESE CARS.

3 DAYS AFTER BUYING MY 04 PRIUS BOTH OF MY LOW BEAMS AND ONE OF MY HIGH BEAMS WENT OUT. A FEW MONTHS AFTER REPLACING THEM MY DRIVER SIDE LOW BEAM WENT OUT, THE HIGH BEAM WAS FINE. 2-3 MONTHS AFTER RE REPLACING THE DRIVER'S SIDE LOW BEAM THE PASSENGER SIDE LOW BEAM WENT OUT. I HAVE HAD TO REPLACE MY HEADLIGHTS ABOUT 5 TIMES EACH (10 TIMES TOTAL DRIVER+PASSENGER SIDE) I HAD MY GROUND WIRES CHECKED, I CONSTANTLY CHECK MY FUSES, MECHANICS SAY NOTHING IS WRONG WITH MY LIGHTING, BUT HERE I AM WITH THIS REOCCURRING ISSUE. I HAVE BEEN USING THE SAME KIND OF BULBS BECAUSE I CAN'T IMAGINE SPENDING MORE MONEY ON A BULB TO HAVE IT BLOW OUT IN A COUPLE OF MONTHS (XTRAVISION 9003 12.8V,55/60W HALOGEN). APPARENTLY MY REVERSE LIGHTS AREN'T WORKING EITHER, AND ONE OF MY LICENSE PLATES LIGHTS IS OUT (I THINK THE PLATE LIGHT IS PROBABLY NOT CONNECTED TO THE OTHER LIGHTS GOING OUT) ZI DO THINK THE REVERSE LIGHTS ARE A RELATED PROBLEM THOUGH. MY LIGHTS GO OUT BOTH IN MOTION AND STATIONARY (I COULDN'T TELL YOU IF IT HAPPENS ON TURNS OR NOT, BUT IT CAN HAPPEN ON HIGHWAYS), SOMETIMES I CAN GET THEM TO COME BACK ON BY GIVING A LITTLE SMACK, BUT USUALLY A FEW DAYS AFTER THAT THEY OFFICIALLY BURN OUT. I'VE RESEARCHED THIS PROBLEM ON PRIUS FORUMS AND APPARENTLY I AM NOT THE ONLY PRIUS OWNER EXPERIENCING THIS WITH THE SAME FREQUENCY, THERE ARE NO RECALLS FOR THIS ISSUE, YET SO MANY OF US ARE EXPERIENCING THIS, WHICH IS QUITE DETRIMENTAL TO OURS AND OTHERS SAFETY. I WAS DRIVING MY 2004 TOYOTA PRIUS ON THE HIGHWAY THERE WERE 5 PASSENGERS IN THE CAR INCLUDING ME. SUDDENLY THERE WAS A LOUD ALARM IN THE CAR. IT WAS A LOUD BEEP. THE BREAKS COMPLETELY FAILED. WHEN I STEPPED ON THE BREAKS THEY DID NOT REACT. I WAS GOING 65 MILES AN HOUR ON THE HIGHWAY AND THANKFULLY WE WERE NOT GOING DOWN HILL. I USED THE ENGINE BRAKE AND PRESSED REALLY HARD ON THE BREAKS TO BRING THE CAR TO A STOP ON AN OFF-RAMP. SINCE I WEIGHT 300 POUNDS I COULD PUT ALL MY WEIGHT INTO THE BRAKES AND FINALLY BROUGHT IT TO A STOP. IF MY WIFE WAS DRIVING WE WOULD HAVE CRASHED. THE WHOLE EPISODE TOOK LIKE 3 MINUTES. IT WAS AGONIZING BECAUSE I THOUGHT WE WERE GOING TO CRASH AGAINST OTHER CARS BECAUSE I HAD NO WAY OF INFORMING THEM THAT I HAD NO BRAKES. THE IDEA OF GOING IN THE RAMP WAS TRYING TO MINIMIZE THE DAMAGE TO OTHER PEOPLE. THE ALARM CONTINUES WHEN I TURNED ON THE CAR. I TOOK THE THE CAR TO THE DEALERSHIP AND THEY SAID THAT THE ACTUATOR WAS THE CULPRIT. THEY TOLD ME IT WAS A MATTER OF TIME. I WAS SITTING ON A FAULTY PART. THEY SAID IT WAS ON A WARRANTY AND NOT ON A RECALL SO THEY COULD HAVE ONLY FIXED IT IF IT FAILED AND I WAS TOO LATE FOR THE FIX. THE WARRANTY EXPIRED ON DECEMBER 31, 2017. IN OTHER WORDS, MY 147,700 MILES TOYOTA IS ESSENTIALLY TOTALED. THE FIX COST MORE THAN THE VALUE OF THE CAR. THEY TOLD ME TO DISABLE THE ABS AND I WOULD BE OK. A MAVIS BRAKE MECHANIC TELLS ME NOT RISK IT. I ALWAYS TOOK MY TOYOTA TO SERVICE AT TOYOTA DEALERSHIPS. THEY NEVER MENTIONED THE RECALL IN SPITE OF THE FACT THAT THE BRAKE LIGHT WAS ON. WHEN THEY PERFORMED THE OIL CHANGE THE LIGHT WOULD TURN OFF FOR SEVERAL WEEKS AND THEN TURN ON. I COULD HAVE KILLED MYSELF IN THE CAR. I FEEL LUCKY THAT NOTHING HAPPENED. THIS OCCURRED TO ME YESTERDAY WHILE TRAVELING 60 MPH IN 94 F HEAT ON I-76 E JUST BEFORE THE 676 / 76 SPLIT. I WAS TRAVELING IN THE FAR LEFT LANE WHEN THE CAR SUDDENLY LOST POWER. LOOKING AT THE DASH I NOTICED THE SHIFTER HAD MOVED ITSELF INTO N (NEUTRAL), IT WOULD NOT MOVE BACK INTO DRIVE. I APPLIED MY HAZARD LIGHTS AND BEGAN FORCED MY WAY RIGHTWARD ACROSS THREE LANES TO THE SHOULDER, SPEED RAPIDLY DECREASING AND SEEMINGLY MORE SLUGGISHLY THAN A NORMAL COAST. I THINK THE EMERGENCY BRAKE WAS AUTOMATICALLY BEING APPLIED OR I WAS FIGHTING A BRAKING SYSTEM. ONE LANE FROM THE SHOULDER, A UPS TRUCK WAS HEADED FOR ME FROM BEHIND. LUCKILY, HE WAS ABLE TO SWERVE INTO THE SHOULDER AND PASS ME THERE AS HE COULD NOT MOVE LEFT INTO OTHER TRAFFIC. WE ONLY NARROWLY AVOIDED A COLLISION WITH ME OR THE ELEVATED BARRIER ON THE SIDE OF THE ROAD. I HAD ONLY 10 MPH LEFT AND NO ABILITY TO REDIRECT MY VEHICLE. ONCE ON THE SIDE OF THE EXPRESSWAY, I WAS ABLE TO GOOGLE THIS WELL-KNOWN ISSUE WITH THE VEHICLE. I LET THE VEHICLE COOL OFF, TURNED OFF THE AIR CONDITIONING, THOUGH DIDN'T WANT TO RISK TURNING THE VEHICLE OFF. I WAS CONCERNED I WOULD HAVE TO WAIT UNTIL NIGHTFALL TO GET BACK INTO TRAFFIC (STILL FLOWING AROUND 60 MPH AND A VERY NARROW SHOULDER) LUCKILY DURING A SMALL TRAFFIC BACKUP UP THE ROAD I WAS ABLE TO RE-ENTER TRAFFIC AT A MODEST SPEED. THE BRAKE ACTUATOR IN MY CAR, WHICH IS A KNOWN PROBLEM IN THESE CARS GAVE OUT AS CONFIRMED BY BOTH TOYOTA AND AN INDEPENDENT MECHANIC SHOP. THIS IS A VERY DANGEROUS SAFETY ISSUE THAT COULD GET EASILY CAUSE AN ACCIDENT. IT IS UNACCEPTABLE.

THE DEALER WAS NOT CONTACTED. THE MANUFACTURER WAS NOTIFIED AND DID NOT ASSIST. THE FAILURE MILEAGE WAS 83,772.
